An alternative to DBScribe, which supports many more platforms, and more comprehensively at that, is SqlSpec from Elsasoft, available here: http://www.elsasoft.org

Another alternative that people might want to consider is SchemaToDoc at http://www.schematodoc.com. It documents (HTML and Word output) SQL Server and Oracle databases. It also lets a user annotate database tables and fields; the output can include these user annotations.
